Have Problems? A stolen or lost passport is a very serious matter!! If you have lost your current valid passport, please report it immediately to the U.S. Department of State using the following contact information: - Toll free: 1-877-487-2778 (TTY 1-888-874-7793) - Operators are available 8 a.m. to 10 p.m., ET, Monday-Friday, excluding Federal holidays. Once you've reported the loss of your passport, the State Department will immediately invalidate it. If you find your passport at a later time, you won't be able to use it to travel. Instead, send it to: U.S. Department of State

Getting A Replacement
So now that your lost passport has been invalidated, you will need to apply for a new one! You will need to file forms DS-11 and DS-64.
Follow the instructions found here for filing Form DS-11.
Along with that, prepare
Form DS-64, Statement Regarding a Stolen or Lost Passport
You'll need to apply in person at a
passport acceptance facility
if you wish to obtain a new passport after your previous one has been lost or stolen.
